The providers and staff at Queen Creek Health Center provide premier medical care and a welcoming environment to residents and visitors of Queen Creek and surrounding communities. We strive to optimize the health and well-being of those ages 3 through adulthood of all abilities, incomes, and backgrounds.
Who We Are
Queen Creek Health Center is a family-owned primary care practice dedicated to giving each patient a medical home. Led by Nicole Read, APRN-Rx, FNP-C, who brings with her years of both private practice and public nursing experience and oversight. Her husband, Kevin Read, handles operations thanks to extensive business management experience both within and without the healthcare industry. On any given visit, you might also meet daughter, Maile, at the front desk, mother-in-law, Sharlene overseeing human resources, and, on occasion, oldest daughter, Kaitlyn, who is completing her fourth year of pre-med studies at Oregon State University. There truly is a family passion for health and well-being!
Our talented and caring support staff are well-known in the community and just as passionate as Nicole and her family about delivering personalized medicine. QCHC provides the entire range of primary care from physical exams and preventive care to treating illnesses and chronic conditions. Whether you are 3 or 93, we help guide you to your best health.

From your first appointment, our entire team will provide you with high-quality and cost-effective medical care. We have same-day sick appointment availability and strive to get all patients scheduled within 2-3 business days. We are committed to evidence-based care, educating patients to optimize health and well-being.
Among the services available at both locations are immunizations, sports and mission physicals, wellness exams; chronic disease management; preventive care, and sick visits. We also offer online scheduling and a secure patient portal for the accessibility of your records.
In the coming months, we’ll be announcing new wellness and aesthetic services that will make Queen Creek Health Center a one-stop shop for all of our patients.
