The Queen Creek Health Center medical care practitioners are dedicated to providing premier healthcare in one location for residents and visitors of Queen Creek, Gilbert, Chandler, San Tan Valley, and surrounding communities.
You and those you care for each have unique needs, from seasonal allergies to chronic illnesses. An important part of our philosophy is to provide as many services as possible in one location.
While there will still be times when you will receive a referral to an outside specialist, our hope is that through regular visits and preventive care, we can provide the majority of your needs in-house.

Primary Care — We provide primary medical care to patients ages 3 through adult of all abilities, incomes, and backgrounds. Our medical office serves residents and visitors from Queen Creek, Chandler, and surrounding communities. More about Primary Care Services
Urgent Care — If you have an urgent medical need, we can schedule your appointment quickly, usually on the same day you call us so that you can get the care you need as soon as possible.
Care for Chronic Conditions — To ensure continuity of care, individuals with an ongoing medical condition should try to see the same primary care practitioner each time. Among the most common chronic conditions are:
Pediatric Care (Ages 3+) — As little ones grow and change, our providers make sure that each child has the best care possible. This includes:
- Well-child care
- Immunizations
- Age-related screenings for physical, mental, and developmental health
- Sick care
- Care for chronic conditions
- School/sports physicals
- Depression or anxiety
We can also identify children with learning disabilities such as ADD/ADHD and provide a specialist referral.

Hormone Replacement Therapy (HRT) for Women — Hormone Replacement Therapy (HRT) is a treatment used to alleviate symptoms caused by hormonal imbalances or deficiencies. Whether you’re experiencing menopause, low testosterone, or seeking gender-affirming care, HRT can play a crucial role in improving your quality of life. Annual Exam — Even seemingly healthy adults should see a primary care practitioner once a year for a physical exam, also known as a preventive visit. Young children, pregnant women, the elderly, and individuals with chronic conditions, may need to be scheduled more frequently.
Medicare Wellness Visit — If you’ve had Medicare Part B for longer than 12 months, you are entitled to a yearly “Wellness” visit to develop or update a personalized prevention plan to help prevent disease and disability, based on your current health and risk factors. Your provider may also perform a cognitive impairment assessment. You pay nothing for this visit if your healthcare provider accepts the assignment, however, if additional tests or services are needed during the visit, those aren’t covered under the preventive benefits. Physicals — QCHC and CFC also conduct school, work, sports, and mission physicals for all ages. Our providers will complete all needed forms and we offer convenient, cash-pay options for each. Please note that we do not currently offer Disability Physicals/Exams.
Vaccinations & Immunizations — We’ll help you and your family members stay updated on recommended vaccines not only during flu season but all year long. Vaccines are often associated with seniors and young children, but the reality is that almost everyone can benefit from obtaining appropriate vaccinations throughout their lives.

Minor Procedures — A minor surgical procedure typically includes those that can be safely performed in an outpatient setting, without the use of general anesthesia or the need for respiratory assistance.
Telehealth Visits — Individuals who are immune-compromised or unable to travel can see their healthcare provider from the comfort of home with a consultation using their smartphone, tablet, or computer. During your consultation, your provider can talk to you about your health concerns, work on a care plan, and if needed, provide a prescription order while you remain safe in your home. Telehealth visits are the same cost as a standard in-office visit.
